  pages 5-10 of this document are the pages you will actually print out, fill in, and turn in to the state.  The first 4 pages are a checklist and instructions.  Several pages must be notarized prior to sending them in.  The first year you file, you must also include each child's birth certificate.  You must turn these in a receive a letter back from the state (which usually takes several weeks) before you can begin homeschooling.  These forms must be completed again each summer before school begins in the fall.
